Cheerleading

In order for you to tryout, you MUST:

1. Have a completed “Consent and Indemnity” form, signed by a parent and brought to your FIRST scheduled tryout or dropped off to Ms. Paradis prior to tryouts. Any player who does not have this form will NOT be allowed to tryout. (See FORMS tab above)
2. Have a current physical on file with the school nurse. Please check with Mrs. Fleming (school nurse) to make sure you are up to date. (See FORMS tab above)
3. Have a completed Medical History Update Form turned in to your coach or Ms. Paradis prior to tryouts.
