TRYOUT REQUIREMENTS:

1. You MUST have the following documents completed in order to try out. All forms can be found under the 'FORMS' tab above.
   • Consent & Indemnity Form: If this form is NOT completely filled out (including insurance information), your student-athlete will not be able to participate in tryouts. If you do not have insurance, please contact Danielle Paradis (Athletic DIrector) for information.
   • Medical Update Form
   • Sports Physical
     • The school nurse MUST have a physical on file before a student will be allowed to try out.
     • If you have already participated on a GHHMS team this Fall or Winter you do not need another physical. You only need ONE in your time at GHHMS.
2. Come dressed appropriately and prepared with proper equipment for your sport.
